Create an Action Plan to Make Your Organization More Accessible and Inclusive

The Inclusive Provider Self-Audit Tool is designed to help you assess and improve the inclusivity of your healthcare organization to people with disabilities – specifically individuals with intellectual and developmental disabilities (IDD). The goal of this tool is for you to develop a concrete action plan that enables your organization to make meaningful progress towards inclusion each year.

This tool is designed for a wide variety of healthcare clinics, offices, and facilities. The tool consists of seven domains (labeled on the tool’s tabs):

  • Staff Training
  • Communication
  • Physical Space
  • Operations
  • Policies and Procedures
  • Autonomy and Support
  • Specialty Areas.

Within each domain, the tool asks a series of questions about the current state of accessibility and inclusion within your organization and practices. Each question has a corresponding effort level required to complete the task. Those action items that are legally required, such as by the Americans with Disabilities Act (ADA), are so indicated. When answering each question, you may choose one of five pre-populated timeframes to address those action items that you have prioritized.

Choosing a specific timeframe automatically adds the needed action items to an action plan to make your organization more inclusive. To finalize the action plan, you should set goals around specific dates to completion and actionable steps to address each item.
